Babylon GP at Hand takes extra care to make sure you’re getting the support you need.

As well as providing all the traditional GP services, our app enables you to access a whole extra level of care whenever and wherever you may need it:

  • Digital consultations at a time and place of your choice

  • 24/7 access to a clinician, 365 days a year

  • Access to a dedicated Personalised Care & Navigation Team

Personalised Care & Navigation Team

Our team supports you to make decisions about your care.

We aim to help you feel confident and empowered to manage your own health.

The team includes Social Prescribers, Health & Wellbeing Coaches, Care Navigators and a Care Coordinator.

Our Social Prescribers can give you information and connect you with services to benefit your health and wellbeing. Some of the support you may be signposted to includes:

  • Housing advice
  • Financial guidance
  • Self-help for mental health conditions
  • Support for carers
  • Community activities and volunteering
  • Support with social isolation
  • Food bank vouchers

Our Care Navigators provide a concierge service and can make sure the right appointments are booked at the right time for you. They’ll direct you to the most appropriate contact in our GP at Hand network, giving you easy access to the assessments and advice you need.

Our Care Coordinator can help to manage any existing referrals and streamline your care. They’ll direct you to the most appropriate contact in our GP at Hand network, giving you easy access to the assessments and advice you need.

Working collaboratively with you, our Health & Wellbeing Coaches can help you create an action plan, with practical steps towards a specific health goal.

If you’re a GP at Hand patient, our clinicians can refer you to the Personalised Care & Navigation with your consent.

Please note that our Personalised Care & Navigation members are non-clinical and do not replace clinical care. We are available Monday-Friday.
