Our care coordination team are here for you

Babylon GP at Hand cares for all types of patients - and, if you happen to have more complex health and social needs, we take extra care to make sure you’re getting the support you need.

As well as providing all the traditional GP services, our app enables you to access a whole extra level of care whenever and wherever you may need it:

  • Digital consultations wherever you are

  • 24/7 access, 365 days a year, to a GP (usually within 2 hours)

  • All consultations are recorded, so can be downloaded and shared

  • Access to a dedicated complex care team

The Care Coordination team

Our Care Coordination team supports patients who have been identified as having complex health or social care needs.

The Care Coordination team aim to improve the health and well-being of these individuals through patient-led care planning and ongoing monitoring. The care planning process aims to promote independence and help patients to confidently manage their own health. To achieve this goal, the patient and the Care Coordination team will work together to identify the patient’s support needs, develop and implement an action plan and help monitor the patient’s progress.

The Care Coordination Team have a dedicated team of GP’s, nurses and care coordinators whom work with our in-house multidisciplinary team to collaboratively support complex patients.

People managed by the Care Coordination team:

  • Receive a carefully tailored Care Plan which is reviewed every six months. Every care plan is different and is individually tailored to the needs of each patient.
  • Have regular contact with their allocated Care Coordinator who can support patients in managing aspects of their health, including liaising with secondary care teams, monitoring referrals and signposting to specialist services.
  • Are assigned a Care Coordination GP to ensure better continuity of care

Who we treat

The Care Coordination team manage a diverse range of patients with a number of different needs. This can include;

  • People with complex mental health issues
  • People with complex physical health issues
  • People who are deemed vulnerable due to safeguarding concerns or homelessness

This list is not exhaustive and there are many different scenarios where a person could benefit from being managed by the Care Coordination team.
